The Physician Orders for Life Sustaining Treatment (POLST) form is a physician order form that communicates medical information about a patient's end of life care wishes. It is intended for use with patients not expected to live beyond one year. The form covers resuscitate/do not resuscitate, medical interventions, antibiotic use, artificial nutrition and comfort measures. This form may be ordered by hospital members only.
